If you’re an electrical apprentice from another IBEW local and you’ve been sent to a job in our area, fill this out before your first day. It gives the training center and the union hall everything they need to set you up, so you don’t have to come in to do it in person. Then one stop in Beaver for your credit union account.

About ten minutes. It asks who you are, your home local and training center, your safety cards, and who to call in an emergency.

Apprentices only. If you’re a Journeyman coming here to work, don’t use this form — call the IBEW Local Union 712 hall at 724-775-0969 and they’ll take care of you.

Do this before your first paycheck

Open a credit union account. Stop at the union hall first.

While you work here, your employer pays your vacation money into an account at West Penn P&P Federal Credit Union in Beaver. No account, and that money has nowhere to go.

  1. Go to the IBEW Local Union 712 hall at 217 Sassafras Lane, Beaver, and fill out the paperwork there.
  2. Take it to the credit union at 1089 Third Street, Beaver, and open the account. Bring photo ID and $50.

What happens after you send it

The training center sets you up

Our office creates your record so your time here is on file. If you need to sign in to anything while you’re here, we’ll send you what you need.

The union hall gets your information

The same form goes to IBEW Local Union 712 in Beaver, so the hall has what it needs for your referral. You don’t have to send it twice.

We get in touch if anything is missing

If a card, a number or a date is missing, the office will call or email you. Otherwise you’re set. The hall will tell you which contractor you’re going to and when you start.

Have these handy

The form goes faster if you have these in front of you:

  • Your home local number and your membership ID
  • Your home training center’s name and a phone number or email for them
  • Your current year and period in the program, and your hours to date if you know them
  • Your drug-free card and any safety certifications (OSHA, NFPA 70E, CPR, first aid)
  • An emergency contact’s name and phone number
